Your first appointment will take about an hour and a half - this will include your evaluation and treatment for the first time.  It is advisable to arrive 10 to 15 minutes early in order to complete the paperwork required for your first visit.  If you wish to save time  click on the link for our intake form - it can be printed and completed prior to your first visit. 

When your visit is complete you will schedule your next appointment  . 

We do ask if you have to cancel or reschedule please try to do so at least 24 hours in advance in order to free up space for other patients.

You will require:     

•  Your Insurance Card
• Any Test Results you may have (MRI, Post Op)
• Prescription from your Doctor
• Referral if needed for your Insurance Company
• A list of your current medications
• The copay required by your Insurance Company
• A parent if you are under 18

What to Wear :

Dress in comfortable , loose fitting clothing that will let you move easily and allow our professionals to evaluate your condition .  Sweatpants and sneakers seem to be the most popular choice for our patients.
